SQL Query For VM/Physical in SAM

Need a SQL script that lists all servers in SAM, sorted by whether they're physical or VM, and creates a table with columns showing node name, IP address, and host (if VM). I don't need a count - I need node names and IPs.  I've checked all over Thwack, and have come up empty.
